You should fill out an FL401 form (https://www.gov.uk/government/publications/form-fl401-application-for-a-non-molestation-order-occupation-order). There is no fee.

You should also write a witness statement, giving details of why you need the order and what it should include.

Submit the form and witness statement to the court in triplicate. The court will set a hearing date, likely within two weeks. You will then need to arrange service on the respondent. No order will be made until the hearing date.
